AHS Daily COVID-19 Checklist

Please complete this daily checklist prior to attending school.

COVID-19 Daily Checklist

If a student answers YES to any of the questions, they MUST NOT be allowed to attend school. Individuals with fever, cough, shortness of breath or loss of sense of smell/taste are required to isolate for 10 days unless they receive a negative COVID-19 test and are feeling better.

Primary Symptoms Guide

Primary Symptoms include the following symptoms that are new, worsening or not related to other known causes:

  • Fever
  • Cough
  • Shortness of Breath
  • Loss of Sense of Smell / Taste

Secondary Symptoms Guide

Secondary Symptoms include the following symptoms that are new, worsening or not related to other known causes:

  • Chills
  • Sore Throat
  • Painful Swallowing
  • Runny or Stuffy Nose 
  • Feeling Unwell or Very Tired
  • Nausea / Vomiting / Diarrhea
  • Unexplained Loss of Appetite
  • Muscle or Joint Aches
  • Headache
  • Conjunctivitis (commonly known as pink eye)
